I get asked about gut testing at least once per week.
Which test. Which one is worth the money. And which one my doctor will actually look at.
I am no stranger to these questions, because I’ve taken many of these tests myself.
So let’s talk about Tiny Health, because it’s the one landing in my inbox lately, and because there’s a version of this review that would just list features and take your money. That’s not what this is.
Here’s what I want you to walk away knowing: what the test actually measures, what it genuinely cannot tell you, and whether you’re the person who should buy one.

And way back when, like when Tiny Health first came on the market, the founder, Cheryl Sew Hoy, reached out. At the time, I simply did not think I could take on one more gut health or gut healing review for the site.
But I kept my eye on the company. I even had a full call with Cheryl several months back so I could learn all about the company and the product itself.
Cheryl has paved the way for so many incredible things and advances in the gut microbiome testing sphere. I knew I had to share.
But first, what is Tiny Health?
Tiny Health is an at-home microbiome test. You collect a stool sample with a swab, mail it back in a prepaid envelope, and get a report. [Similar to others I have done.]
The company started with babies. Founder Cheryl Sew Hoy built it after a C-section, when she wanted to know her baby’s gut trajectory and couldn’t find a straight answer anywhere. They’ve since expanded to tests for children, adults, pregnancy, and vaginal health.
That origin story matters more than it sounds like it should. I’ll come back to it.
The company is based in Austin. Samples run through a CLIA-certified and CAP-accredited lab. Their advisory team includes Dr. Elisa Song (Stanford, NYU, and UCSF-trained integrative pediatrician) as Chief Medical Officer and Dr. Emeran Mayer, who wrote The Mind-Gut Connection.
How the test actually works
The mechanism is the part most reviews skip, and it’s the part that determines whether the results mean anything.
Tiny Health uses deep shotgun metagenomic sequencing. In plain language: instead of looking for a pre-selected list of microbes, the lab sequences all the microbial DNA in your sample and then matches it against reference databases.
Compare that to the other two methods you’ll run into:
- 16S rRNA sequencing reads one shared bacterial gene. Cheap, fast, and it usually can’t get past genus level. It tells you Bacteroides is there. It won’t tell you which one.
- qPCR (what GI-MAP uses) targets a fixed panel of organisms with pre-designed DNA probes. Very sensitive for the things on the list. Blind to everything off it.
Shotgun sequencing sees more. Tiny Health says its tests screen for over 120,000 microbes, including bacteria, fungi, parasites, viruses, and archaea, down to 0.005% relative abundance.
Tiny Health vs other gut tests
I built this because “which test should I get” is the wrong question until you know what each one is built to do. They’re not competing versions of the same thing.
Here’s how the major testing methods actually compare. Specs below are drawn from Tiny Health’s published technology comparison, cross-checked against each company’s own materials. Worth saying plainly: the source chart is Tiny Health’s, so the framing favors them. I’ve noted where competitors would argue back.

The short version:
Tiny Health and Floré both use shotgun sequencing, but only Tiny Health reports strain-level detail and age-specific reference ranges. GI-MAP costs more and usually needs a practitioner, but gives absolute counts, which matters if you’re chasing a specific low-level pathogen. 16S tests like Ombre are cheapest at $119 to $129 and least detailed, stopping at genus or family level. None of the stool tests diagnose SIBO.
What Tiny Health can screen for, by age
This is the part I wish more companies published. What the test measures depends on how old you are, because a 6-month-old and a 40-year-old are not the same organism.
Adults get biomarkers and associations for constipation, IBS, Long COVID, PCOS, SIBO, and urinary tract infections.
Pregnancy and trying to conceive adds gestational diabetes, preterm birth risk via the vaginal microbiome, and preeclampsia.
Babies 0 to 6 months get the widest panel: eczema, asthma, atopic march, food allergies, colic, cow’s milk protein allergy, obesity later in life, sleep, type 1 diabetes, and viral respiratory infections.
Babies 6 to 12 months keep most of that, minus eczema and colic.
Toddlers 1 to 3 narrow to food allergies, sleep, type 1 diabetes, and constipation.
Children 3 to 18 narrow further, to sleep and constipation.
Read that trajectory. The panel shrinks as your child gets older, and that’s the whole argument for testing early. The window when the gut is most responsive is also the window when there’s the most to measure.
Note: Tiny Health splits these into biomarkers (validated measurements for that age) and associations (signatures emerging research links to a condition). Not the same thing, and the distinction matters when you read your report.
Can Tiny Health detect SIBO?
Here’s the thing nobody selling you a gut test wants to say out loud.
A stool test cannot diagnose SIBO.
SIBO is bacterial overgrowth in the small intestine. A stool sample tells you about the large intestine. Those two environments have very little in common.
The gold standard is culture of jejunal aspirate at 10³ CFU/mL or higher, which is invasive and requires a gastroenterologist. The practical alternative is a hydrogen and methane breath test. The North American consensus calls a hydrogen rise of 20 ppm or more above baseline within 90 minutes positive, and methane at 10 ppm or higher positive at any point. Mayo Clinic is honest that even breath testing is less specific than we’d like.
Now, here’s where Tiny Health is more careful than most companies in this space, and it’s worth understanding.
Their reports separate Conditions Biomarkers from Conditions Associations. Biomarkers are the measurements they’ve validated for a given age group. Associations are microbial signatures that emerging research has linked to a condition.
SIBO sits in the associations column, available for adults and for pregnancy/TTC. So the test can flag patterns in your gut that research associates with SIBO. It cannot tell you that you have it.
That’s a meaningful signal, and it’s also not a diagnosis. Both of those things are true at once, and I’d rather you understand the difference than buy on a misunderstanding.
If you want a SIBO answer, get a breath test. If you want to know whether your microbiome is carrying signatures associated with SIBO, plus what else is going on down there, that’s what this test does.

The report breaks into a summary with an expert highlight, key insights calibrated to your age, a full microbiome breakdown categorized as beneficial, unfriendly, variable, or unknown, a conditions section flagging microbiome signatures associated with certain conditions, and an action plan with diet, supplement, probiotic, and lifestyle suggestions.
That coaching session is the part I’d actually weigh in the price. A 40-page microbiome report you don’t understand is worth nothing. Someone walking you through it is worth quite a lot.
The HSA/FSA eligibility is the underrated line item. If you have funds sitting in an HSA, the math on this changes completely.
What happens after your results come back
Nobody covers this part, and it’s the entire difference between interesting data and an actual change.
You get the report. You get the action plan. And then you have to do something with it.
The action plan will give you diet suggestions, supplement suggestions, probiotic strains, and lifestyle changes. Some of it will be things you’re already doing. Some of it will be new. Almost none of it works in 3 weeks.
Take the coaching call. Ask which 2 or 3 items matter most, because a list of 15 recommendations is a list nobody finishes. Then pick a window, work the plan, and retest.
That’s the loop: test, change, retest. A single report is a snapshot. Two reports 4 to 6 months apart is information you can actually use.

Where the science is solid, and where it isn’t
I’d rather you buy something with clear eyes than buy something because I sold it well.
Solid: shotgun metagenomics is the better technology for breadth. It reads species and often strain level, catches fungi and parasites and viruses that 16S misses entirely, and aligns to public reference databases (RefSeq, NCBI, GTDB) that anyone can inspect. That transparency is a real advantage over qPCR panels, where companies don’t publish their primer sequences, so nobody outside the lab can verify what a positive result actually means.
Not solid: shotgun sequencing reports relative abundance, meaning percentages of a whole, not absolute counts. That’s a genuine limitation and GI-MAP’s lab makes the point fairly. Their argument is that qPCR catches opportunists like Klebsiella and Candida at 100 to 1,000 cells per gram, where sequencing often needs 100,000 or more to see them. If you’re hunting a specific low-level pathogen, that gap matters.
Also not solid: the field as a whole. There’s no randomized controlled trial showing that microbiome test recommendations beat standard dietary advice for symptom outcomes. Tiny Health says this plainly in their own disclaimer, and I’ll say it too. Their tests are not intended to diagnose or treat disease and are not a substitute for professional medical advice.
I don’t think that makes testing worthless. I think it makes testing a data point, not a verdict.
Who Tiny Health is good for
You’ve been dismissed and you want data. Walking into an appointment with a report changes the conversation. Not because it’s diagnostic, but because it’s specific.
You just finished antibiotics. This is one of the clearest use cases. You know something got disrupted. Testing tells you what and gives you a baseline to rebuild from.
You’re taking probiotics and have no idea if they’re working. This is my favorite reason. Tiny Health makes a point I’ve made for years: more probiotic is not automatically better. If a beneficial species is already high in you, pushing it higher can crowd out diversity. Testing tells you whether the bottle you’ve been buying for 8 months is doing anything at all.
You want a before and after. Test, change things for a few months, retest. That’s the strongest version of what this technology does.
You have kids. Which brings me to the part where Tiny Health is genuinely different.
The family piece
Most gut tests run adult reference ranges. For a baby, that’s not a small technical quibble. It produces wrong conclusions.
A 4-month-old and a 12-month-old have meaningfully different microbiomes. Both are entirely different from a 40-year-old. Tiny Health calibrates its key insights to age, and it’s the first test I’m aware of built for the 0 to 3 window from the start.
The first 1,000 days matter because that’s when immune programming happens, and the gut is at its most malleable. Vaginal birth versus C-section, breastfeeding versus formula, antibiotic exposure. All of it shapes what gets built.

Note: if you’re pregnant, they recommend testing in the first trimester and again in the third. The vaginal microbiome piece is real. Higher Lactobacillus crispatus at 16 weeks has been associated with protection against preterm birth, and women who deliver preterm more often show low protective Lactobacillus and higher levels of bacteria associated with bacterial vaginosis.
Talk to your OB. Bring the data. Don’t replace your OB with the data.

Who this is NOT for

You want a SIBO diagnosis. Covered above. Get a breath test.
You’re in acute crisis. Blood in stool, unexplained weight loss, severe pain. That’s a doctor today, not a swab and a 4-week wait.
You’re going to spiral over the results. Some of you know exactly what I mean. If a report listing “unfriendly” bacteria is going to send you into three weeks of panic-Googling, the test will cost you more than money.
You won’t act on it. The report is only worth the changes you make afterward. If you’re collecting data instead of changing anything, save the money and change something first.
You need results next week. 3 to 4 weeks is a long wait when you feel terrible.
My honest read
Tiny Health is a well-built test using the right technology, from a company that discloses its methods and doesn’t pretend the science is more settled than it is. The coaching session and the age-calibrated pediatric ranges are the two things that separate it from the pack.
It is not a diagnostic. Buy it for the broad picture, not for a verdict. It will not find your SIBO, and it cannot identify which specific foods you’re allergic to.
If you understand that and still want to know what’s living in your gut, so you can stop guessing about probiotics and start tracking change over time, this is a good use of $249. Especially if you have kids.
